• Application Process:Contact via phone or the City of Lakewood website to obtain the registration forms. Once completed and submitted, the citizen will be entered into the system and can call to schedule rides.
• Eligibility Requirements:Residents of Lakewood who are either age 60 or older, and/or those with a disability of any age.
• Payment/Insurance Accepted:Medicaid, Private Pay, Cash, Check, Credit Card. Medicaid = HCBS funds as referred by Jefferson County Human Services.
