The Care Navigator is a care management agency that provides Guardianship, situation/care assessment/recommendations, bill paying, care coordination with physicians/service providers, arranging transitions from home to care communities and from hospitals and nursing homes to return home, coordinating legal needs and estate planning, long term care insurance, end of life care and services related to supporting individuals in their desire to maintain health and remain independent. This is a for-profit corporation. Agency/Program does not provide transportation services.

8:00am - 5:00pm Monday through Friday
