Metro East nursing homes cited for violations leading to death, injuries

April 11--SPRINGFIELD -- Several Metro East nursing homes were cited for serious violations during the first quarter of the year, including one that led to a resident's death, according to information released Friday by the Illinois Department of Public Health.

Rosewood Care Center in Edwardsville was cited in February and March; Jerseyville Nursing and Rehab Center was cited in February; and Bria of Belleville was cited in January. All were listed as "A" violations.

"AA" violations that are involved in a resident's death, while "A" violations deal with conditions that could or has resulted in the "serious probability of death or mental or physical harm. Violations are listed quarterly.

In February, Rosewood Care Center, a 120-bed facility at 6277 Center Grove Road, Edwardsville, was cited with an "A" violation and fined $25,000 for failure to safely transfer a resident, resulting in the death of a resident due to a fall from a mechanical lift.

The facility was also cited in March with an "A" violation and fined $25,000 for failure to provide supervision to prevent falls, resulting in one resident falling and suffering serious injuries.

The facility has requested hearings in both cases.

Also in March, Jerseyville Nursing and Rehab Center, an 111-bed facility at 1001 S. State St., Jerseyville, was cited with an "A" violation and fined $25,000 for failure to prevent a resident from falling from a wheelchair. The resident sustained multiple serious injuries.

The facility waived its right to a hearing and paid $16,250.

Bria of Belleville, a 140-bed facility at 150 N. 27th St., Belleville, was cited with an "A" violation and initially fined $25,000 for failure to provide antibiotic treatment and treatment for potential dehydration, which resulted in a resident being admitted to the hospital with septic shock.

The facility paid a $12,500 fine.
